Notice of Privacy Practices
Effective date: October 2026 · Last updated: October 2026
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Our Commitment
We are required by law to protect the privacy of your health information, to give you this Notice explaining our legal duties and privacy practices, and to follow the terms of the Notice currently in effect.
We are also required to notify you if a breach occurs that compromises the privacy or security of your health information.
How We May Use & Disclose Your Information
For treatment. To provide, coordinate and manage your care. For example, your provider may review your bloodwork before prescribing, or share information with another clinician involved in your treatment.
For payment. To obtain payment for services, including billing you or, where applicable, a health plan.
For health care operations. To run the practice — quality review, staff training, licensing, audits and business management.
Appointment reminders and follow-up. To contact you about appointments, results and treatment alternatives that may be of interest to you. Tell us your preferred contact method and we will use it.
Others involved in your care. We may share relevant information with a family member, friend or carer you have identified, or in an emergency where it is in your best interests.
Business associates. Contractors who perform services for us — our records system, billing and IT support — may handle your information. Each is contractually required to protect it.
As required or permitted by law. Including public health activities, reporting abuse or neglect, health oversight, judicial and administrative proceedings, law enforcement in defined circumstances, coroners and medical examiners, organ donation, serious threats to health or safety, workers' compensation, and specialised government functions.

Substance Use Disorder Records
Certain substance use disorder treatment records receive protection under federal law (42 CFR Part 2) that is stricter than HIPAA. Where we hold such records, those stricter rules apply.
Substance use disorder treatment records obtained from a Part 2 program cannot be used or disclosed in civil, criminal, administrative or legislative proceedings against you without your written consent or a proper court order.
Your written consent is required before such records are used or disclosed for most purposes, including any fundraising communication. You may opt out of fundraising communications at any time, and choosing to do so will not affect your treatment.
Once information has been disclosed with your permission, it may be redisclosed by the recipient and may no longer be protected by HIPAA.
Other federal and state laws may also impose restrictions stricter than HIPAA. Where they do, we follow the stricter rule.

If you believe your privacy rights have been violated, you may complain to us or directly to the Secretary of the U.S. Department of Health and Human Services. You will not be penalised or retaliated against for filing a complaint.
